Criminal Record Check

As we stress safety throughout all aspects at Layritz, we require all coaches to get annual criminal record checks.

All managers, coaches, and directors are required to complete criminal records checks.  If you have not submitted one yet, please do so immediately.

You can download and print it directly off the Saanich Police web site under CONSENT FOR POLICE INFORMATION CHECK.

Applicants must produce one piece of photo identification and one piece of identification verifying name and date of birth.   

The fee for this form is waived for volunteer positions of non-profit organizations when accompanied with a request on official letterhead.
